Form5 Prosthetics is a 501(c)(3) nonprofit that develops affordable, custom 3D-printed adaptive products for people with limb differences. Founded by Aaron Westbrook after he printed his first device as a 15-year-old high school student, the organization combines research and development with hands-on CO-FAB workshops in which recipients work directly with students and professional mentors. Its work also includes the Nub Club of Ohio, a community of more than 80 individuals with limb differences and their families.

Action ProstheticsFounded by prosthetist Karl Burk in 2008, Action Prosthetics designs and fits customized prostheses for amputees of all ages from its Troy and Greenville offices. Burk’s rehabilitation background and master-carpentry experience inform the practice’s hands-on focus on comfortable fit, with prosthetic work kept on site rather than sent to a central fabrication facility.
